Healthy Smiles Dental Plan
At Parker Family Dental, we’ve put together a dental plan to help offset the cost of dental care for those who don’t have insurance. It’s designed to help people maintain oral health when they are without dental coverage. We hope this will help people who might otherwise feel they can’t afford dental care.
The fee for this plan is $385/year for each adult and $315/year for each child 13 years old and younger. The plan includes 2 cleanings in 12 month period (from the day you sign up), 2 exams, routine diagnostic radiographs (bitewings 1 time per-year and full series 1 time per 5 years) and the child fluoride 1 time per year. In addition all restorative dental work will be done at a discounted rate of 17%.
Exclusions & Limitations
- Patients must not have a balance to start the plan
- Patients must pay the annual fee prior to any restorative treatment
- All services must be paid in full when services are rendered
- There is no co-pay, patients only need to pay their portion of the services
- There is no annual maximum
- The plan is valid for exactly 1 year after purchase
- The plan is non-transferable
- The plan is non-refundable even if you move or fail to keep your appointment
- Cannot be combined with any other insurance coverages
- No further discounts or benefit plan can be used in conjunction
- Discount does not apply to oral care merchandise
- Does not apply to any orthodontic care
